Indications:
HEXIMAX 5 mg is indicated for the symptomatic treatment of all forms of Parkinson’s disease (idiopathic, post-encephalitic, and arteriosclerotic types), and for controlling extrapyramidal side effects caused by neuroleptic (antipsychotic) drugs.
Pharmacology:
Trihexyphenidyl is a centrally acting anticholinergic that works by blocking acetylcholine receptors in the central nervous system. This restores the balance between acetylcholine and dopamine, helping to reduce symptoms like tremor, rigidity, and slow movement.
Dosage & Administration:
Parkinsonism: Initial dose 1 mg/day, gradually increased to 6–10 mg/day in divided doses depending on patient response.
Extrapyramidal symptoms: Usually 1–4 mg/day.
Best taken with or after food to avoid gastrointestinal irritation.
Interaction:
May enhance effects of other anticholinergic drugs. Use with caution alongside CNS depressants or alcohol. May antagonize the effects of parasympathomimetic agents and reduce levodopa efficacy at high doses.
Contraindications:
Known hypersensitivity to Trihexyphenidyl
Narrow-angle glaucoma
Gastrointestinal or urinary tract obstruction
Side Effects:
Common: Dry mouth, blurred vision, constipation, dizziness
Others: Nausea, urinary retention, nervousness
Rare: Confusion, hallucinations, increased heart rate
Pregnancy & Lactation:
Should only be used if clearly needed and prescribed by a physician. Use during pregnancy and lactation requires caution as safety is not fully established.
Precautions & Warnings:
Use cautiously in elderly patients and those with cardiac, hepatic, or renal conditions
Avoid activities requiring mental alertness (e.g., driving)
Prolonged use requires monitoring for signs of mental confusion or visual problems
