Indications:
Indicated for the treatment of type 2 diabetes mellitus, especially in overweight patients, when diet and exercise alone do not result in adequate glycemic control.
Pharmacology:
Metformin decreases hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization.
Dosage:
Initially 500 mg once or twice daily with meals. Maximum dose: 2000–2550 mg/day in divided doses depending on response and tolerability.
Administration:
Should be taken with meals to reduce gastrointestinal side effects. Do not crush or chew the tablet unless specified as extended-release.
Interaction:
May interact with alcohol, iodinated contrast agents, corticosteroids, beta-blockers, and diuretics. Concomitant use may increase the risk of lactic acidosis or reduce glucose-lowering effect.
Contraindications:
Renal dysfunction, metabolic acidosis, diabetic ketoacidosis, severe liver disease, hypersensitivity to Metformin.
Side Effects:
Common side effects include nausea, vomiting, diarrhea, abdominal discomfort, and metallic taste. Rare but serious side effect: lactic acidosis.
Pregnancy & Lactation:
Generally safe during pregnancy, but insulin may be preferred. Small amounts pass into breast milk; use with caution during lactation.
Precautions & Warnings:
Monitor renal function before and during treatment. Avoid in patients at risk of lactic acidosis. Use with caution in elderly and dehydrated patients.
