Indications:
CARDOTEL 5/40 MG is indicated for the treatment of essential hypertension, especially in patients whose blood pressure is not adequately controlled on monotherapy. It also reduces the risk of cardiovascular events.

Pharmacology:

Telmisartan is an angiotensin II receptor blocker (ARB) that blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II.
Amlodipine is a dihydropyridine calcium channel blocker that inhibits calcium ion influx into vascular smooth muscle and myocardium, leading to vasodilation.
The combination provides additive antihypertensive effects by targeting different pathways.

Dosage & Administration:

One tablet daily, preferably at the same time each day, with or without food.
Dose adjustments should be based on clinical response and tolerance.
Not recommended as initial therapy unless clinically justified.

Interaction:

Increased risk of hyperkalemia with potassium supplements or potassium-sparing diuretics (due to Telmisartan).
Caution with other antihypertensives (additive effect).
NSAIDs may reduce the antihypertensive effect.
Grapefruit juice may affect Amlodipine metabolism.

Contraindications:

Hypersensitivity to Telmisartan, Amlodipine, or any component of the formulation
Severe hypotension or cardiogenic shock
Pregnancy (especially 2nd and 3rd trimesters)
Severe hepatic impairment

Side Effects:

Dizziness, peripheral edema, fatigue
Hypotension, flushing
Hyperkalemia (rare)
Rare: Angioedema, renal dysfunction

Pregnancy & Lactation:
Contraindicated during pregnancy—ARBs can cause fetal harm. Use with caution in lactating mothers as Amlodipine is excreted in breast milk.
