Indications:
CARDOTEL 5/40 MG is indicated for the treatment of essential hypertension, especially in patients whose blood pressure is not adequately controlled on monotherapy. It also reduces the risk of cardiovascular events.

Pharmacology:

  • Telmisartan is an angiotensin II receptor blocker (ARB) that blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II.

  • Amlodipine is a dihydropyridine calcium channel blocker that inhibits calcium ion influx into vascular smooth muscle and myocardium, leading to vasodilation.
    The combination provides additive antihypertensive effects by targeting different pathways.

Dosage & Administration:

  • One tablet daily, preferably at the same time each day, with or without food.

  • Dose adjustments should be based on clinical response and tolerance.

  • Not recommended as initial therapy unless clinically justified.

Interaction:

  • Increased risk of hyperkalemia with potassium supplements or potassium-sparing diuretics (due to Telmisartan).

  • Caution with other antihypertensives (additive effect).

  • NSAIDs may reduce the antihypertensive effect.

  • Grapefruit juice may affect Amlodipine metabolism.

Contraindications:

  • Hypersensitivity to Telmisartan, Amlodipine, or any component of the formulation

  • Severe hypotension or cardiogenic shock

  • Pregnancy (especially 2nd and 3rd trimesters)

  • Severe hepatic impairment

Side Effects:

  • Dizziness, peripheral edema, fatigue

  • Hypotension, flushing

  • Hyperkalemia (rare)

  • Rare: Angioedema, renal dysfunction

Pregnancy & Lactation:
Contraindicated during pregnancy—ARBs can cause fetal harm. Use with caution in lactating mothers as Amlodipine is excreted in breast milk.

Precautions & Warnings:

  • Monitor blood pressure, renal function, and electrolytes regularly

  • Use cautiously in patients with heart failure or liver/kidney impairment

  • Avoid abrupt withdrawal

  • Caution in patients with volume depletion or severe aortic stenosis
