Indications:
NEBICARD 2.5MG is indicated for the treatment of essential hypertension and mild to moderate chronic heart failure in elderly patients.
Pharmacology:
Nebivolol is a selective beta-1 adrenergic receptor blocker with vasodilating properties due to nitric oxide release. It reduces heart rate and myocardial contractility, leading to reduced cardiac output and blood pressure.
Dosage & Administration:
Hypertension: Starting dose is 2.5 to 5 mg once daily; maximum dose up to 10 mg/day.
Heart Failure: Start at 1.25 mg daily, gradually titrate based on response.
Dose adjustment may be required in elderly or renal-impaired patients.
Administer orally, with or without food, preferably at the same time daily.
Interaction:
May enhance the effects of other antihypertensives.
Use cautiously with calcium channel blockers (e.g., verapamil, diltiazem).
NSAIDs may reduce its effectiveness.
Risk of bradycardia with digitalis glycosides.
Contraindications:
Severe bradycardia
Heart block greater than first degree
Decompensated cardiac failure
Hypersensitivity to Nebivolol
Severe hepatic impairment
Side Effects:
Headache, fatigue, dizziness
Bradycardia, hypotension
Gastrointestinal discomfort
Dyspnea, peripheral edema (in heart failure patients)
Pregnancy & Lactation:
Use in pregnancy only if clearly needed. Beta-blockers may reduce placental perfusion and cause fetal/neonatal side effects. Use during lactation is not recommended.
Precautions & Warnings:
Monitor heart rate and blood pressure regularly
Use cautiously in patients with diabetes, bronchospastic diseases, or peripheral vascular disorders
Avoid abrupt discontinuation
Not recommended in patients with hepatic dysfunction
