Indications:
• Acute coronary syndrome (unstable angina, NSTEMI, STEMI) to reduce myocardial infarction and stroke;
• Recent MI, recent stroke, or established peripheral arterial disease to lower risk of recurrent events
Pharmacology:
Clopidogrel is a prodrug that irreversibly inhibits P2Y₁₂ ADP receptors on platelets, preventing activation and aggregation. Platelet inhibition appears ~2 hours after a dose; steady state reached Day 3–7 with daily 75 mg
Dosage & Administration:

ACS: Initial loading dose 300 mg (four 75 mg tablets) then maintenance 75 mg once daily

Recent MI/stroke/PAD: 75 mg once daily without loading dose

Administer orally with or without food; swallow whole
Interaction:

Increased bleeding risk with NSAIDs, warfarin, SSRIs/SNRIs

CYP2C19 inhibitors (omeprazole, esomeprazole) reduce efficacy—avoid or use pantoprazole

Avoid repaglinide coadministration
Contraindications:
Active pathological bleeding (peptic ulcer, intracranial hemorrhage), hypersensitivity to clopidogrel
Side Effects:
Common: bleeding (including GI), diarrhea, abdominal discomfort, bruising, skin rash
Rare: thrombotic thrombocytopenic purpura (TTP) .
Pregnancy & Lactation:
Limited human data; no clear evidence of harm during pregnancy, but benefit vs risk should guide use. Safety in breastfeeding unclear
Precautions & Warnings:
Use with caution in hepatic impairment, renal disease, trauma/surgery risk of bleeding, discontinue ≥7 days before elective surgery
Monitor if on CYP2C19 inhibitors.
Therapeutic Class:
Antiplatelet agent – Thienopyridine derivative.
