Indications:
Prazopress ER is indicated for the treatment of hypertension, congestive heart failure, and Raynaud's phenomenon. It may also be used in benign prostatic hyperplasia as an off-label use.
Pharmacology:
Prazosin is a selective alpha-1 adrenergic receptor blocker. It causes peripheral vasodilation by relaxing smooth muscles in blood vessels, thereby reducing blood pressure. Its extended-release formulation allows for sustained effect with fewer doses.
Dosage & Administration:
The usual starting dose is 2.5 mg once daily, preferably at bedtime to reduce the risk of first-dose hypotension. Dosage may be adjusted based on patient response.
Interaction:
May interact with other antihypertensives, diuretics, beta-blockers, and PDE5 inhibitors (e.g., sildenafil). Alcohol may enhance the blood pressure-lowering effect.
Contraindications:
Hypersensitivity to Prazosin or other quinazolines. Caution is advised in patients with orthostatic hypotension.
Side Effects:
Common side effects include dizziness, headache, fatigue, drowsiness, and palpitations. Rarely, it may cause syncope especially after the first dose.
Pregnancy & Lactation:
Use in pregnancy only if clearly needed and prescribed. Caution should be exercised during lactation as data is limited.
Precautions & Warnings:
Caution in patients with existing low blood pressure, elderly patients, and those with renal impairment. Monitor blood pressure regularly. Avoid abrupt discontinuation.
