Indications:
Glucomet is indicated for:
Management of type 2 diabetes mellitus
Improving glycemic control in patients inadequately controlled by diet and exercise alone
Often used in combination with other oral hypoglycemic agents or insulin
Pharmacology:
Metformin reduces hepatic glucose production (gluconeogenesis), decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization, thus lowering blood glucose levels without causing hypoglycemia.
Dosage & Administration:
Typical starting dose: 500 mg twice daily with meals
Dose may be gradually increased to a maximum of 2,000–2,500 mg per day based on clinical response
Should be taken with meals to reduce gastrointestinal side effects
Tablets should be swallowed whole with water
Interaction:
May interact with cimetidine, furosemide, nifedipine, and certain contrast agents
Alcohol increases risk of lactic acidosis
May enhance hypoglycemic effect of other antidiabetic agents
Contraindications:
Severe renal impairment
Acute or chronic metabolic acidosis including diabetic ketoacidosis
Hypersensitivity to Metformin
Conditions predisposing to hypoxia (e.g., severe cardiac or respiratory failure)
Side Effects:
Common: Gastrointestinal upset (nausea, diarrhea, abdominal discomfort)
Rare but serious: Lactic acidosis (medical emergency)
Vitamin B12 deficiency with long-term use
Pregnancy & Lactation:
Use during pregnancy only if clearly needed. Metformin crosses the placenta but no teratogenic effects reported. Use during lactation is generally considered safe but monitor infant.
Precautions & Warnings:
Monitor renal function before and during treatment
Avoid use in patients with significant renal impairment
Temporarily discontinue before iodinated contrast imaging procedures
Encourage adherence to diet and exercise program
