Indications:
RABE 20 MG is indicated for the treatment of:
Gastroesophageal reflux disease (GERD)
Duodenal and gastric ulcers
Zollinger-Ellison syndrome
Erosive esophagitis
Maintenance therapy after healing of erosive esophagitis
Helicobacter pylori eradication (in combination with antibiotics)
Pharmacology:
Rabeprazole is a proton pump inhibitor (PPI) that suppresses gastric acid secretion by inhibiting the H⁺/K⁺ ATPase enzyme system at the secretory surface of the gastric parietal cells. This reduces both basal and stimulated acid production, aiding in healing and symptom relief.
Dosage & Administration:
GERD: 20 mg once daily for 4–8 weeks
Gastric ulcer: 20 mg once daily for 4–6 weeks
Duodenal ulcer: 20 mg once daily for 4 weeks
H. pylori eradication: 20 mg twice daily with antibiotics for 7–10 days
Should be taken before meals, preferably in the morning
Swallow whole; do not crush or chew
Interaction:
May reduce absorption of drugs dependent on gastric pH (e.g., ketoconazole, atazanavir)
May increase levels of digoxin, cyclosporine, and warfarin
Use with caution with methotrexate—can delay clearance
Antacids can be co-administered without affecting efficacy
Contraindications:
Hypersensitivity to Rabeprazole or substituted benzimidazoles
Severe hepatic impairment without medical supervision
Concomitant use with rilpivirine-containing products
Side Effects:
Headache, nausea, diarrhea, abdominal pain
Flatulence, constipation
Long-term use may lead to vitamin B12 deficiency, hypomagnesemia, or osteoporosis-related fractures
Rare: Skin rash, liver enzyme abnormalities
Pregnancy & Lactation:
Use only if clearly needed during pregnancy. Animal studies have not shown harm, but human data is limited. Rabeprazole is excreted in breast milk—use caution while breastfeeding.
Precautions & Warnings:
Prolonged use may increase risk of gastric malignancy
Monitor magnesium levels with long-term use
Evaluate for H. pylori infection in recurrent ulcers
Consider dose adjustment in hepatic impairment
