Indications:
Glucomet XR is indicated for the management of type 2 diabetes mellitus, particularly in overweight patients, when diet and exercise alone do not provide adequate glycemic control. It may be used as monotherapy or in combination with other antidiabetic agents.
Pharmacology:
Metformin decreases hepatic glucose production, reduces intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization. It does not cause insulin secretion and thus does not usually lead to hypoglycemia. The XR (extended-release) formulation allows for once-daily dosing with sustained effect.
Dosage & Administration:
The usual starting dose is 750 mg once daily with the evening meal. Dosage may be adjusted in increments of 750 mg weekly based on blood glucose response. Maximum recommended dose is 2000 mg per day. Tablets should be swallowed whole and not crushed or chewed.
Interaction:
May interact with alcohol (risk of lactic acidosis), diuretics, corticosteroids, beta-blockers, and iodinated contrast media. Monitor renal function when used with nephrotoxic agents.
Contraindications:
Contraindicated in patients with renal impairment (eGFR <30 mL/min/1.73 m²), metabolic acidosis, dehydration, sepsis, or in cases of recent myocardial infarction. Also contraindicated during radiologic procedures involving iodinated contrast.
Side Effects:
Common side effects include gastrointestinal discomfort such as nausea, diarrhea, abdominal pain, and loss of appetite. Rare but serious side effect is lactic acidosis, especially in patients with kidney dysfunction.
Pregnancy & Lactation:
Generally considered safe during pregnancy for managing diabetes under medical supervision. Use with caution in lactating mothers as it may be excreted in small amounts in breast milk.
Precautions & Warnings:
Monitor renal function regularly. Avoid excessive alcohol intake. Discontinue temporarily before radiologic procedures using contrast media and in conditions predisposing to hypoxia or dehydration.
Therapeutic Class:
Biguanides; Oral hypoglycemic agents.

Store below 30°C in a dry place away from moisture and heat. Keep out of reach of children.
