Indications:
Bopam is indicated for:

  • Treatment of moderate to severe anxiety and panic disorders

  • Nervous tension and restlessness

  • Short-term management of insomnia associated with anxiety

  • As premedication before minor surgical or diagnostic procedures

Pharmacology:
Bromazepam is a benzodiazepine that enhances the effect of the inhibitory neurotransmitter GABA (gamma-aminobutyric acid) at GABA-A receptors. This results in anxiolytic, sedative, muscle-relaxant, and anticonvulsant effects.

Dosage & Administration:

  • Adults: Initial dose is 1.5–3 mg two to three times daily

  • Maximum daily dose typically does not exceed 6–18 mg depending on patient response

  • Should be taken orally with water, with or without food

  • Use lowest effective dose for the shortest duration

Interaction:

  • Additive sedative effect with alcohol, opioids, antihistamines, and other CNS depressants

  • Caution with antipsychotics, antidepressants, anticonvulsants

  • Avoid with strong CYP3A4 inhibitors (e.g., ketoconazole, ritonavir)

Contraindications:

  • Hypersensitivity to Bromazepam or other benzodiazepines

  • Severe respiratory insufficiency

  • Myasthenia gravis

  • Sleep apnea syndrome

  • Acute narrow-angle glaucoma

  • Severe hepatic impairment

Side Effects:

  • Common: Drowsiness, fatigue, dizziness, sedation

  • Less common: Confusion, coordination problems, blurred vision

  • Serious (rare): Dependence, withdrawal symptoms, memory impairment, behavioral changes

Pregnancy & Lactation:
Contraindicated in pregnancy, especially during the first trimester due to potential teratogenic effects. Also contraindicated during breastfeeding as the drug passes into breast milk and may cause sedation in the infant.

Precautions & Warnings:

  • Risk of dependence and withdrawal symptoms if used long-term or abruptly discontinued

  • Caution in elderly (risk of falls and sedation)

  • Avoid alcohol and other sedatives

  • Not for use in children unless specifically prescribed
