Indications:
Cardex is indicated for the treatment of:

  • Hypertension (high blood pressure)

  • Mild to moderate congestive heart failure (CHF)

  • Left ventricular dysfunction following myocardial infarction

  • Angina pectoris (off-label)

Pharmacology:
Carvedilol is a non-selective β-adrenergic blocker with α1-blocking activity. It reduces heart rate, myocardial contractility, and peripheral vascular resistance. This leads to decreased blood pressure and reduced workload on the heart, making it effective in heart failure and post-infarction support.

Dosage & Administration:

  • Initial dose for heart failure: 3.125 mg twice daily

  • For hypertension: Start with 6.25 mg once/twice daily (if needed)

  • Should be taken with food to reduce orthostatic hypotension

  • Dose to be adjusted gradually by physician

  • Do not stop suddenly — taper off under medical supervision

Interaction:

  • Additive effects with other antihypertensives or beta-blockers

  • Caution with calcium channel blockers (e.g., verapamil, diltiazem)

  • May enhance effects of insulin or oral antidiabetics

  • Avoid with CYP2D6 inhibitors (e.g., fluoxetine)

Contraindications:

  • Severe bradycardia, heart block > first degree

  • Decompensated heart failure requiring IV inotropes

  • Bronchial asthma or other reactive airway disease

  • Liver impairment

  • Hypersensitivity to Carvedilol

Side Effects:

  • Common: Dizziness, fatigue, hypotension, bradycardia

  • Less common: Diarrhea, weight gain, hyperglycemia

  • Serious (rare): Bronchospasm, AV block, liver enzyme elevation

Pregnancy & Lactation:
Use only if clearly needed during pregnancy; not recommended during breastfeeding without physician guidance.

Precautions & Warnings:

  • Monitor blood pressure and heart rate regularly

  • Use cautiously in diabetic patients (may mask hypoglycemia symptoms)

  • Avoid alcohol and abrupt discontinuation

  • Caution in patients with peripheral vascular disease or COPD
