Indications:
• Panic disorder with or without agoraphobia
• Lennox‑Gastaut syndrome (petit mal variant), akinetic and myoclonic seizures
• Absence seizures in patients unresponsive to succinimides
Pharmacology:
Clonazepam enhances GABAergic neurotransmission via positive allosteric modulation of GABAₐ receptors, exhibiting anticonvulsant, anxiolytic, sedative, and muscle‑relaxant activity
Dosage:
Seizure disorders (adults): start ≤ 1.5 mg/day in 3 divided doses; increase by 0.5–1 mg every 3 days up to a max of 20 mg/day.
Panic disorder: start at 0.25 mg twice daily, titrate to 1 mg/day after 3 days
Pediatrics: 0.01–0.03 mg/kg/day (max 0.05 mg/kg/day) in divided doses
Administration:
Taken orally, with or without food; swallow whole. IV/IM forms available for status epilepticus in hospital settings .
Interaction:
CNS depressants and alcohol increase sedative effects.
Combining with other anticonvulsants may potentiate depressant actions
Contraindications:
Hypersensitivity to benzodiazepines, acute narrow‑angle glaucoma, severe liver disease. Use caution in open‑angle glaucoma with appropriate treatment
Side Effects:
Common: somnolence (≈37%), ataxia (5–10%), dizziness, fatigue, memory impairment, confusion, depression, blurred coordination .
Rare: respiratory depression (esp. in children), blood dyscrasias
Pregnancy & Lactation:
Risk of teratogenicity in pregnancy; use only if benefits outweigh risks. May pass into breast milk; caution advised
Precautions & Warnings:
High habit‑forming potential; avoid abrupt discontinuation due to risk of withdrawal symptoms (e.g., tremor, anxiety, confusion).
Taper dose gradually.
Caution in hepatic/renal impairment; avoid driving until response is known.
Monitor patients on multiple CNS depressants
