Indications:
Mirtaz is indicated for:
Major depressive disorder (MDD)
Anxiety associated with depression
Sleep disturbances and insomnia related to depressive illness
Often used off-label at low doses for sleep support in patients with anxiety or depression
Pharmacology:
Mirtazapine is a noradrenergic and specific serotonergic antidepressant (NaSSA). It enhances central noradrenergic and serotonergic activity by blocking presynaptic α2-adrenergic receptors. It also blocks 5-HT2 and 5-HT3 receptors, which contributes to its anxiolytic and sleep-promoting properties.
Dosage & Administration:
Initial dose: 7.5–15 mg once daily, usually taken at bedtime
Maintenance dose: Can be increased gradually to 30–45 mg daily as needed
Should be taken regularly at the same time each day
Swallow whole with water, with or without food
Interaction:
Increased risk of serotonin syndrome when used with SSRIs, MAOIs, or triptans
CNS depressants (alcohol, benzodiazepines) may enhance sedative effects
May increase levels of phenytoin or carbamazepine
Caution with anticoagulants like warfarin
Contraindications:
Known hypersensitivity to Mirtazapine or any of its components
Concurrent use with or within 14 days of discontinuing MAO inhibitors
Side Effects:
Common: Drowsiness, weight gain, increased appetite, dry mouth, dizziness
Less common: Edema, abnormal dreams, constipation, hypotension
Rare but serious: Blood dyscrasias (e.g., neutropenia), suicidal thoughts (especially in young adults), serotonin syndrome
Pregnancy & Lactation:
Should be used during pregnancy only if clearly necessary. Use with caution during breastfeeding as it may pass into breast milk. Always consult a physician.
Precautions & Warnings:
Monitor mood, behavior, and suicidal tendencies especially in initial weeks
Caution in elderly patients and those with hepatic or renal impairment
May impair alertness; avoid driving or operating heavy machinery
Gradual dose tapering is recommended during discontinuation
