Indications:
Xeldrin is indicated for the treatment of duodenal and gastric ulcers, gastroesophageal reflux disease (GERD), erosive esophagitis, Zollinger-Ellison syndrome, and in combination therapy for Helicobacter pylori eradication.
Pharmacology:
Omeprazole is a proton pump inhibitor that suppresses gastric acid secretion by specific inhibition of the H⁺/K⁺ ATPase enzyme system in the gastric parietal cell. It is a prodrug that is activated in the acidic environment of the stomach. This leads to long-lasting inhibition of acid secretion.
Dosage & Administration:
Adults: 20 mg once daily before breakfast, typically for 4–8 weeks depending on the condition.
For H. pylori eradication: Used in combination with antibiotics.
Capsule should be swallowed whole with water; do not crush or chew.
Interaction:
May interact with warfarin, diazepam, ketoconazole, clopidogrel (reduces its effect), digoxin, and antifungal agents. Omeprazole may reduce the absorption of drugs that require an acidic medium.
Contraindications:
Contraindicated in patients with known hypersensitivity to omeprazole or other substituted benzimidazoles. Avoid use in patients with severe liver impairment without monitoring.
Side Effects:
Common side effects include headache, abdominal pain, diarrhea, nausea, vomiting, and flatulence. Long-term use may result in vitamin B12 deficiency, hypomagnesemia, and increased risk of bone fracture.
Pregnancy & Lactation:
Category C in pregnancy; should be used only if clearly needed. Omeprazole is excreted in breast milk; use during lactation should be done with caution.
Precautions & Warnings:
Prolonged treatment may mask symptoms of gastric malignancy. Use with caution in patients with liver disease, osteoporosis, or those at risk of electrolyte disturbances.
