Montelukast is a selective and orally active leukotriene receptor antagonist that inhibits the cysteinyl leukotriene receptor (CysLT₁). Cysteinyl leukotrienes (LTC₄, LTD₄, LTE₄), products of arachidonic acid metabolism, are released from various cells, including mast cells and eosinophils. These leukotrienes and receptor occupation are linked to asthma and allergic rhinitis pathophysiology, contributing to airway edema, smooth muscle contraction, and altered cellular activity in the inflammatory process. Arokast is indicated for the prophylaxis and chronic treatment of asthma, acute prevention of exercise-induced bronchoconstriction (EIB), and relief of symptoms of allergic rhinitis (AR), including both seasonal and perennial forms. The recommended dosage for adults and adolescents aged 15 years and older is one 10 mg tablet once daily, taken in the evening. It is important to note that Arokast is not intended for the reversal of acute bronchospasm and should not be used as a substitute for fast-acting inhalers.
