Indications:

  • Type 2 Diabetes Mellitus in adults

  • Especially for overweight patients where diet, exercise, and monotherapy do not provide adequate glycemic control

  • Can be used as initial therapy or as step-up therapy

Pharmacology:
This combination contains:

  • Glimepiride: A sulfonylurea that stimulates insulin secretion from the pancreatic beta cells.

  • Metformin: A biguanide that decreases hepatic glucose production, reduces intestinal absorption of glucose, and improves insulin sensitivity.

Together, they work synergistically to maintain blood glucose levels in patients with type 2 diabetes.

Dosage:

  • Common dose: 1 mg Glimepiride + 500 mg Metformin once or twice daily

  • Dosage should be individualized based on blood glucose response

  • Maximum dose should not exceed Glimepiride 8 mg and Metformin 2000 mg per day

Administration:

  • Take with meals to reduce gastrointestinal side effects

  • Swallow whole with water

  • Maintain regular mealtime and physical activity

Interaction:

  • Increased hypoglycemia risk with NSAIDs, insulin, MAOIs, beta-blockers

  • Decreased Metformin effect with corticosteroids, diuretics, and estrogen

  • Alcohol may increase risk of lactic acidosis

Contraindications:

  • Type 1 diabetes

  • Diabetic ketoacidosis

  • Severe renal or hepatic impairment

  • Congestive heart failure

  • History of lactic acidosis

  • Pregnancy and lactation

Side Effects:

  • Hypoglycemia

  • Nausea, vomiting

  • Diarrhea

  • Abdominal discomfort

  • Metallic taste

  • Rare: Lactic acidosis (from Metformin), blood disorders, hepatic dysfunction

Pregnancy & Lactation:

  • Contraindicated in pregnancy; use insulin instead

  • Should be avoided during breastfeeding

Precautions & Warnings:

  • Monitor renal function regularly

  • Risk of hypoglycemia—avoid skipping meals

  • Be cautious in elderly and malnourished patients

  • Educate patients on recognizing lactic acidosis symptoms: muscle pain, weakness, rapid breathing
