To reduce the risk of stroke in hypertensive patients with left ventricular hypertrophy
Losartan is an angiotensin II receptor blocker (ARB). It selectively blocks the binding of angiotensin II to the AT1 receptor, resulting in vasodilation, reduced secretion of vasopressin, and reduced production and secretion of aldosterone — leading to lowered blood pressure and reduced workload on the heart.
Adults (hypertension): 50 mg once daily; may be increased to 100 mg/day
Diabetic nephropathy: Initial dose 50 mg once daily, may increase to 100 mg
Can be taken with or without food
Dosage should be adjusted based on patient response and tolerability
May increase serum potassium levels when taken with potassium-sparing diuretics or supplements
NSAIDs may reduce the antihypertensive effect
Risk of lithium toxicity if taken with lithium
Avoid combination with aliskiren in diabetic patients
Hypersensitivity to Losartan or any component of the formulation
Pregnancy (especially 2nd and 3rd trimester)
Severe hepatic impairment
Common: Dizziness, fatigue, upper respiratory infections, nasal congestion
Serious: Hypotension, hyperkalemia, renal impairment, angioedema (rare)
Pregnancy: Contraindicated, especially in second and third trimesters due to risk of fetal injury or death
Lactation: Not recommended; unknown whether losartan is excreted in breast milk
Monitor renal function and potassium levels
Caution in patients with hepatic impairment or volume depletion
Avoid during pregnancy
May cause symptomatic hypotension, especially after first dose in volume-depleted patients
Angiotensin-II Receptor Blockers (ARBs)
