Type 2 Diabetes Mellitus
Can be used alone or in combination with metformin, sulfonylureas, or insulin when diet and exercise alone do not provide adequate glycemic control
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor. It works by increasing levels of incretin hormones, which help regulate blood glucose by increasing insulin release and decreasing glucagon secretion in a glucose-dependent manner.
Recommended dose: 5 mg once daily, with or without food
No dose adjustment needed for renal or hepatic impairment
Can be taken at any time of the day but preferably at the same time each day
Risk of hypoglycemia increases when used with sulfonylureas or insulin
Co-administration with rifampin (a potent CYP3A4 inducer) may reduce linagliptin exposure
Minimal drug-drug interaction due to primarily non-renal excretion
Hypersensitivity to linagliptin or any component of the formulation
Type 1 diabetes or for the treatment of diabetic ketoacidosis
Common: Nasopharyngitis, cough, back pain
Rare but serious: Hypersensitivity reactions like angioedema, pancreatitis, urticaria, and exfoliative skin conditions
Pregnancy: Use only if clearly needed and prescribed by a doctor
Lactation: It is unknown whether linagliptin is excreted in human milk; caution is advised
Monitor for signs of pancreatitis
Not a substitute for insulin in insulin-requiring patients
Monitor blood glucose regularly
Use caution in patients with a history of pancreatitis
