Indications:
OFURAN SR 100 MG is indicated for the treatment and prophylaxis of urinary tract infections (UTIs) caused by susceptible strains of Escherichia coli, Enterococci, Staphylococci, and certain Gram-negative organisms.

Pharmacology:
OFURAN SR contains Nitrofurantoin in a sustained-release formulation. It works by interfering with bacterial carbohydrate metabolism by inhibiting acetyl coenzyme A, disrupting cell wall synthesis, and causing bacterial death. The sustained release ensures prolonged therapeutic action in the urinary tract.

Dosage:

  • Treatment of UTIs: 100 mg twice daily for 5–7 days.

  • Prophylaxis: 100 mg once daily at bedtime.
    Dosage may vary based on physician’s advice and patient condition.

Administration:
Should be taken with food to enhance absorption and reduce gastrointestinal side effects. Swallow whole with water; do not crush or chew the tablet.

Interaction:
Nitrofurantoin’s absorption may be decreased by antacids containing magnesium trisilicate. Probenecid and sulfinpyrazone can increase nitrofurantoin levels in the blood and may increase toxicity.

Contraindications:
Contraindicated in patients with anuria, oliguria, significant renal impairment (creatinine clearance <60 mL/min), history of jaundice or hepatic dysfunction associated with nitrofurantoin, and in neonates under 1 month.

Side Effects:
May include nausea, vomiting, loss of appetite, headache, dizziness, or urine discoloration (brown). Rare but serious effects include pulmonary reactions, hepatitis, and peripheral neuropathy.

Pregnancy & Lactation:
Can be used during pregnancy except near term (38–42 weeks) due to risk of hemolytic anemia in the neonate. Excreted in breast milk in small amounts—caution advised during breastfeeding.

Precautions & Warnings:

  • Use with caution in elderly patients and those with kidney problems.

  • Monitor for signs of pulmonary toxicity during prolonged therapy.

  • Discontinue if signs of peripheral neuropathy appear.
