Pharmacology:
Metsa XR contains Metronidazole in an extended-release (XR) formulation.
It works by entering the microbial cell and generating free radicals that damage DNA, leading to the death of anaerobic bacteria and protozoa.
The extended-release form provides sustained plasma concentration, allowing less frequent dosing and improved compliance.
Dosage:
Common adult dose: 500 mg once or twice daily, based on the infection severity.
Duration: Typically 5–10 days, but should be determined by a physician.
Do not crush or chew the tablet as it may interfere with extended-release mechanism.
Administration:
Take the tablet orally, preferably with food to minimize gastrointestinal discomfort.
Swallow whole with water.
Maintain a regular dosing schedule for effective treatment.
Interaction:
Avoid alcohol during and 48 hours after treatment to prevent disulfiram-like reactions.
Can interact with:
Warfarin – may increase bleeding risk
Cimetidine – may raise metronidazole levels
Enzyme-inducing drugs like phenytoin – may lower effectiveness
Lithium – risk of lithium toxicity
Contraindications:
Known hypersensitivity to Metronidazole or related nitroimidazole compounds
History of blood disorders or CNS disorders (e.g., epilepsy)
Caution in severe liver disease
Side Effects:
Common: Nausea, metallic taste, dizziness, headache, diarrhea
Rare: Peripheral neuropathy (prolonged use), dark urine, allergic skin reactions
Serious: Seizures, encephalopathy (very rare)
Pregnancy & Lactation:
Pregnancy: Use in 1st trimester should be avoided unless necessary. In 2nd/3rd trimesters, use only if clearly needed (Category B).
Lactation: Metronidazole is secreted in breast milk. Either avoid breastfeeding during treatment or consult a physician for alternatives.
