Pharmacology:

  • Metsa XR contains Metronidazole in an extended-release (XR) formulation.

  • It works by entering the microbial cell and generating free radicals that damage DNA, leading to the death of anaerobic bacteria and protozoa.

  • The extended-release form provides sustained plasma concentration, allowing less frequent dosing and improved compliance.


Dosage:

  • Common adult dose: 500 mg once or twice daily, based on the infection severity.

  • Duration: Typically 5–10 days, but should be determined by a physician.

  • Do not crush or chew the tablet as it may interfere with extended-release mechanism.


Administration:

  • Take the tablet orally, preferably with food to minimize gastrointestinal discomfort.

  • Swallow whole with water.

  • Maintain a regular dosing schedule for effective treatment.


Interaction:

  • Avoid alcohol during and 48 hours after treatment to prevent disulfiram-like reactions.

  • Can interact with:

    • Warfarin – may increase bleeding risk

    • Cimetidine – may raise metronidazole levels

    • Enzyme-inducing drugs like phenytoin – may lower effectiveness

    • Lithium – risk of lithium toxicity


Contraindications:

  • Known hypersensitivity to Metronidazole or related nitroimidazole compounds

Side Effects:

  • Common: Nausea, metallic taste, dizziness, headache, diarrhea

Pregnancy & Lactation:

  • Pregnancy: Use in 1st trimester should be avoided unless necessary. In 2nd/3rd trimesters, use only if clearly needed (Category B).

  • Lactation: Metronidazole is secreted in breast milk. Either avoid breastfeeding during treatment or consult a physician for alternatives.
