Indications:
Glucomin 500 mg is indicated for the treatment of type 2 diabetes mellitus, particularly in overweight patients when dietary management and exercise alone do not result in adequate glycemic control. It may be used as monotherapy or in combination with other oral antidiabetic agents or insulin.

Pharmacology:
Metformin, the active ingredient in Glucomin, is a biguanide antihyperglycemic agent. It works by decreasing hepatic glucose production, decreasing intestinal absorption of glucose, and improving insulin sensitivity by increasing peripheral glucose uptake and utilization.

Dosage:

  • Adults: The usual starting dose is 500 mg twice a day or 850 mg once a day, taken with meals. The dose may be increased gradually based on patient response and tolerability.

  • Maximum dose: The maximum recommended daily dose is 2550 mg, divided into multiple doses.

Administration:
Take orally with meals to reduce gastrointestinal side effects. Swallow the tablet whole; do not crush or chew.

Interaction:
Metformin may interact with certain medications, including:

  • Cimetidine: May increase metformin levels.

  • Diuretics: May increase the risk of lactic acidosis.

  • Insulin or sulfonylureas: May increase the risk of hypoglycemia.

  • Iodinated contrast agents: Use caution; may increase the risk of lactic acidosis.

Contraindications:

  • Known hypersensitivity to metformin or any component of the formulation.

  • Severe renal impairment (eGFR <30 mL/min/1.73m²).

  • Acute or chronic metabolic acidosis, including diabetic ketoacidosis.

Side Effects:
Common side effects include:

  • Gastrointestinal disturbances: nausea, vomiting, diarrhea, abdominal discomfort.

  • Metallic taste.

  • Rare: lactic acidosis, vitamin B12 deficiency with long-term use.

Pregnancy & Lactation:

  • Pregnancy: Use during pregnancy should be considered only if the potential benefit justifies the potential risk to the fetus.

  • Lactation: Metformin is excreted in breast milk. Caution should be exercised when administered to a nursing woman.

Precautions & Warnings:

  • Monitor renal function regularly.

  • Use with caution in patients with hepatic impairment.

  • Discontinue use before and for 48 hours after administration of iodinated contrast agents.

  • Elderly patients may be at greater risk for lactic acidosis.
