Indications:
Pandura 1 mg is indicated for:

Panic Disorder: With or without agoraphobia, characterized by unexpected panic attacks and associated concerns.

Seizure Disorders: Including Lennox-Gastaut syndrome (petit mal variant), akinetic and myoclonic seizures, and absence seizures unresponsive to succinimides.
Pharmacology:
Clonazepam, the active ingredient in Pandura, is a benzodiazepine that enhances GABAergic neurotransmission, leading to anticonvulsive, sedative, muscle-relaxing, and anxiolytic effects. It suppresses spike and wave discharges in absence seizures and decreases the frequency of seizures.
Dosage & Administration:
Dosage should be individualized based on patient response.

Adults with Panic Disorder:

Start with 0.25 mg twice daily.

After 3 days, increase to 1 mg/day as needed.

Adults with Seizure Disorders:
-
Begin with 1.5 mg/day in divided doses.
-
Increase by 0.5–1 mg every 3 days to a maximum of 20 mg/day.

Pediatric Patients:
-
0.01–0.03 mg/kg/day in divided doses, not exceeding 0.05 mg/kg/day.

Interaction:
Clonazepam does not significantly affect the pharmacokinetics of phenytoin, carbamazepine, or phenobarbital. However, caution is advised when used with other CNS depressants due to potential additive effects.
Contraindications:

Hypersensitivity to benzodiazepines.

Significant liver disease.

Acute narrow-angle glaucoma.
Side Effects:
Common side effects include:

Drowsiness

Ataxia

Fatigue

Depression

Dizziness
These effects may diminish over time with continued use.
Pregnancy & Lactation:

Pregnancy: Use only if the potential benefit justifies the potential risk to the fetus.

Lactation: Not recommended; Clonazepam is excreted in breast milk.
Precautions & Warnings:

Monitor for signs of depression or suicidal thoughts.

Use with caution in patients with a history of substance abuse.

Gradual dose reduction is recommended to avoid withdrawal symptoms.
