Indications:
NIPROLAC is indicated for:
Chronic Constipation: Effective in softening stools and promoting regular bowel movements.
Hepatic Encephalopathy: Helps reduce elevated blood ammonia levels in patients with liver disease.
Disturbances of Intestinal Flora: Useful in restoring healthy gut flora, especially after antibiotic therapy.
Pharmacology:
Lactulose is a synthetic disaccharide metabolized in the colon by saccharolytic bacteria, producing low molecular weight organic acids (mainly lactic acid). This process lowers the pH of the colon contents and promotes water retention by an osmotic effect, increasing peristaltic activity. Lactulose is minimally absorbed, making its pharmacokinetics less relevant to its therapeutic action.
Dosage & Administration:
Dosage should be individualized based on patient response.
Adults:
Constipation: Initially 15–30 ml daily; maintenance dose: 10–25 ml daily.
Hepatic Encephalopathy: 30–45 ml three to four times daily.
Children:
1–5 years: 5–10 ml daily.
6–14 years: 10–15 ml daily.
Infants: 2.5–5 ml daily.
Note: Dosage may be adjusted to produce two to three soft stools per day.
Interaction:
No significant drug interactions have been reported. However, excessive use may lead to electrolyte imbalance, which can enhance the effects of cardiac glycosides.
Contraindications:
Hypersensitivity to Lactulose or any component of the formulation.
Patients on a galactose-free diet.
Suspected intestinal obstruction.
Side Effects:
Common side effects may include:
Flatulence
Abdominal discomfort
Nausea
Diarrhea (especially with high doses)
These effects are usually mild and transient.
Pregnancy & Lactation:
Pregnancy: Category B. Studies show no adverse effects; however, use only if clearly needed.
Lactation: Considered safe; minimal systemic absorption reduces the risk to nursing infants.
Precautions & Warnings:
Use with caution in diabetic patients, as Lactulose contains galactose and lactose.
Monitor electrolyte levels during prolonged therapy.
Overuse may lead to diarrhea and electrolyte imbalance.
