Indications:
Lijenta M 1000 mg is indicated for the treatment of type 2 diabetes mellitus in adults when diet, exercise, and monotherapy do not provide adequate glycemic control. It combines two medications—Linagliptin and Metformin—for improved effectiveness.

Pharmacology:
This combination tablet contains:

  • Linagliptin, a DPP-4 inhibitor that increases incretin levels to enhance insulin release and reduce glucagon levels after meals.

  • Metformin, a biguanide that decreases glucose production in the liver and increases insulin sensitivity.

Together, they lower blood glucose levels more effectively than either alone.

Dosage:
Usually taken once or twice daily with meals, depending on blood glucose levels and medical advice. Dosage must be personalized by a physician.

Administration:
Take with meals to reduce gastrointestinal side effects. Swallow the tablet whole with water. Do not crush or chew.

Interaction:
Alcohol intake, iodinated contrast agents, and some drugs like corticosteroids or beta-blockers may interfere with blood sugar control. Combining with insulin or sulfonylureas may increase hypoglycemia risk.

Contraindications:
Contraindicated in patients with:

  • Renal impairment

  • Metabolic acidosis (including diabetic ketoacidosis)

  • Hypersensitivity to Linagliptin or Metformin

Side Effects:

  • Common: Nausea, diarrhea, flatulence, and abdominal discomfort (mainly due to Metformin)

  • Others: Hypoglycemia (when combined with insulin), headache, upper respiratory infections, or rarely, lactic acidosis (serious)

Pregnancy & Lactation:
Should be used during pregnancy only if clearly necessary. Caution is advised during lactation as Metformin passes into breast milk.

Precautions & Warnings:

  • Monitor kidney function regularly.

  • Do not use in patients with severe liver or kidney problems.

  • Avoid alcohol.

  • Stop medication temporarily before major surgery or radiological procedures involving contrast dye.
