Indications:
Montiva 4 mg is indicated for:

  • Prophylaxis and chronic treatment of asthma in children aged 2 to 5 years

  • Acute prevention of exercise-induced bronchoconstriction (EIB) in children aged 2 years and older

  • Relief of symptoms of allergic rhinitis (seasonal and perennial) in children aged 2 to 5 years

Pharmacology:
Montelukast is a selective leukotriene receptor antagonist that binds to the cysteinyl leukotriene receptor (CysLT1). By inhibiting the action of leukotrienes, it reduces inflammation and bronchoconstriction in the airways, leading to improved asthma control and relief from allergic rhinitis symptoms.

Dosage & Administration:

  • Recommended dose: One 4 mg chewable tablet once daily in the evening.

  • Should be taken at least 1 hour before or 2 hours after food.

  • Not intended for the relief of acute asthma attacks.

Interaction:

  • May interact with phenobarbital, phenytoin, and rifampicin, which are known to induce liver enzymes, potentially reducing the efficacy of Montelukast.

  • Generally, no significant interactions with other commonly used medications for asthma or allergies.

Contraindications:

  • Hypersensitivity to Montelukast or any component of the formulation.

  • Not recommended for children under 2 years of age.

Side Effects:
Common:

  • Headache

  • Abdominal pain

  • Diarrhea

  • Nausea

  • Rash

Serious:

  • Neuropsychiatric events (e.g., agitation, aggression, depression, suicidal thoughts)

Pregnancy & Lactation:

  • Pregnancy: Category B. Use during pregnancy should be considered only if the potential benefit justifies the potential risk to the fetus.

  • Lactation: Montelukast is excreted in breast milk. Caution should be exercised when administered to a nursing woman.

Precautions & Warnings:

  • Not intended for the relief of acute asthma attacks.

  • Should not be used as a substitute for inhaled or oral corticosteroids.

  • Monitor for neuropsychiatric symptoms, especially in children.

  • Discontinue use if neuropsychiatric symptoms occur.
