Cataplexy associated with narcolepsy
Clomipramine, a tricyclic antidepressant, inhibits the reuptake of serotonin and norepinephrine in the brain, enhancing neurotransmitter activity and improving mood and behavior in patients with depression or OCD.
Adults: Usually 25 mg 2–3 times daily, gradually increased if needed
Maintenance dose: Often 50–75 mg daily
Should be taken with food to reduce gastric irritation
Dosage should be individualized and gradually adjusted
Increased risk of serotonin syndrome with SSRIs and MAOIs
Enhances the effects of CNS depressants and alcohol
May interact with anticholinergic agents, antihypertensives, and anticoagulants
Caution with QT-prolonging drugs
Recent myocardial infarction
Concurrent use with monoamine oxidase inhibitors (MAOIs)
Hypersensitivity to clomipramine or other tricyclic antidepressants
History of seizures
Common: Dry mouth, constipation, drowsiness, dizziness, blurred vision
Less common: Weight gain, sexual dysfunction, urinary retention
Rare: Cardiac arrhythmias, seizures, confusion (especially in elderly)
Pregnancy: Use only if clearly needed; may pose risk in the third trimester
Lactation: Excreted in breast milk; use with caution
Monitor for suicidal thoughts, especially in early treatment
Caution in patients with cardiovascular disease, epilepsy, glaucoma, and urinary retention
Gradual withdrawal is advised to avoid discontinuation symptoms
Tricyclic antidepressants (TCA)
Store below 25–30°C in a dry, cool place, away from light. Keep out of reach of children.
