Rimstar 4 FDC is indicated for the initial intensive phase treatment of tuberculosis (TB). It contains a fixed-dose combination of four essential antitubercular drugs, used to reduce the bacterial load quickly and prevent resistance.
Rimstar 4 FDC combines Rifampicin, Isoniazid, Pyrazinamide, and Ethambutol — each with a unique mechanism against Mycobacterium tuberculosis.
Rifampicin inhibits bacterial RNA synthesis
Isoniazid disrupts mycolic acid synthesis in the cell wall
Pyrazinamide lowers intracellular pH, damaging TB bacteria
Ethambutol interferes with cell wall formation by inhibiting arabinosyl transferase
Usually 4 tablets once daily, on an empty stomach, as per body weight
Used for 2 months during the intensive phase of TB treatment
Must be used in combination with continuation phase therapy
Swallow whole with water; do not break or crush
Rifampicin reduces effectiveness of oral contraceptives, warfarin, antiretrovirals
Isoniazid interacts with phenytoin, carbamazepine
Alcohol increases hepatotoxicity risk
Avoid antacids within 2 hours of taking the medicine
Hypersensitivity to any component
Severe liver disease
Optic neuritis (due to ethambutol)
Gout (caution with pyrazinamide)
Common: Nausea, vomiting, fatigue, joint pain, red-orange urine
Serious: Hepatitis, peripheral neuropathy, optic neuritis, rash, hyperuricemia
Use in pregnancy only if clearly necessary
Components pass into breast milk; generally considered safe with monitoring
Pyridoxine supplementation recommended during pregnancy
Monitor liver function and vision regularly
Educate about urine discoloration (harmless)
Supplement Vitamin B6 to prevent neuropathy
Adherence is crucial to avoid resistance development
Avoid alcohol
