Carbamazepine stabilizes hyperexcited nerve membranes, inhibits repetitive neuronal firing, and reduces synaptic propagation of excitatory impulses.
Initial dose usually 100-200 mg twice daily; may be adjusted based on clinical response
Take with food to reduce gastrointestinal irritation
Monitor blood levels for dose optimization
Induces hepatic enzymes, reducing effectiveness of many drugs like oral contraceptives, warfarin, and others
Caution with other CNS depressants
Avoid alcohol during therapy
Hypersensitivity to carbamazepine or tricyclic compounds
Bone marrow suppression or history of agranulocytosis
Recent MAOI use
Pregnancy (unless benefits outweigh risks)
Common: Dizziness, drowsiness, nausea, vomiting
Less common: Ataxia, blurred vision, rash
Serious: Stevens-Johnson syndrome, blood dyscrasias, hepatic dysfunction
Pregnancy: Use only if clearly needed; potential teratogenic effects
Lactation: Caution advised; drug passes into breast milk
Monitor blood counts and liver function regularly
Educate patients on rash or signs of hypersensitivity
Avoid abrupt discontinuation to prevent seizures
