Azithromycin is a macrolide antibiotic that binds to the 50S ribosomal subunit of susceptible microorganisms and inhibits protein synthesis, thereby preventing bacterial growth.
Adults: 500 mg once daily for 3 days (or 500 mg on day 1 followed by 250 mg once daily for the next 4 days)
Can be taken with or without food
Capsule should be swallowed whole with water
Full course must be completed even if symptoms improve earlier
Antacids (especially aluminum/magnesium) can reduce absorption
Increased risk of arrhythmias with QT-prolonging drugs
May enhance effects of warfarin, digoxin, and cyclosporine
Avoid concomitant use with ergot derivatives
Hypersensitivity to azithromycin, erythromycin, or other macrolides
History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use
Severe hepatic impairment
Common: Nausea, vomiting, abdominal discomfort, diarrhea
Less common: Headache, dizziness, skin rash
Rare: QT prolongation, hepatotoxicity, severe allergic reactions
Pregnancy: Category B – use only if clearly needed
Lactation: Excreted in breast milk; monitor infant for GI disturbances
Caution in patients with heart rhythm disorders (especially QT prolongation)
Hepatic function should be monitored during prolonged therapy
Discontinue immediately if severe allergic reaction occurs
