Indications:

Galvus Met 50/850 is indicated for:

  • The treatment of type 2 diabetes mellitus in adults

  • Patients inadequately controlled on metformin alone

  • Patients already being treated with vildagliptin and metformin as separate tablets

Pharmacology:

Galvus Met is a fixed-dose combination of two oral antidiabetic drugs:

  • Vildagliptin: A DPP-4 inhibitor that enhances the levels of incretin hormones, increasing insulin secretion and decreasing glucagon release in a glucose-dependent manner.

  • Metformin: A biguanide that improves insulin sensitivity, reduces hepatic glucose production, and decreases intestinal absorption of glucose.

Together, these agents provide complementary mechanisms to improve glycemic control.

Dosage & Administration:

  • One tablet (50/850 mg) twice daily with meals to reduce gastrointestinal side effects

  • Dosage should be individualized based on patient’s current regimen, effectiveness, and tolerance

  • Not recommended in patients with renal impairment (eGFR <45 mL/min/1.73m²)

Interaction:

  • Alcohol increases risk of lactic acidosis

  • May interact with drugs that impair renal function (e.g., NSAIDs, diuretics, ACE inhibitors)

  • Use with caution alongside other antidiabetic agents to avoid hypoglycemia

  • Contrast agents used in imaging may increase risk of renal impairment

Contraindications:

  • Hypersensitivity to vildagliptin, metformin, or any excipients

  • Renal failure or dysfunction

  • Acute or chronic metabolic acidosis (including diabetic ketoacidosis)

  • Severe hepatic impairment

Side Effects:

  • Common: Nausea, diarrhea, headache, dizziness, upper respiratory tract infection

  • Less common: Hypoglycemia (especially when combined with sulfonylureas), abdominal discomfort

  • Rare: Lactic acidosis, liver enzyme elevation, pancreatitis

Pregnancy & Lactation:

  • Pregnancy: Use only if clearly needed and under medical supervision

  • Lactation: Not recommended due to lack of sufficient data on excretion in breast milk

Precautions & Warnings:

  • Monitor renal and hepatic function periodically

  • Discontinue if metabolic acidosis or unexplained symptoms like muscle pain or fatigue occur

  • Do not use in type 1 diabetes or for diabetic ketoacidosis

  • Regular blood glucose monitoring is essential
