Carbamazepine acts by stabilizing hyperactive neuronal membranes and inhibiting repetitive neuronal firing to control seizures.
Starting dose typically 100 mg once or twice daily, adjusted as needed
Take with food to minimize gastrointestinal side effects
Therapeutic drug monitoring recommended
Induces liver enzymes, affecting metabolism of many drugs including oral contraceptives and warfarin
Increased sedation with CNS depressants
Avoid alcohol during therapy
Hypersensitivity to carbamazepine or related compounds
Bone marrow suppression or history of agranulocytosis
Concomitant MAOI use
Pregnancy unless benefits outweigh risks
Common: Dizziness, drowsiness, nausea, vomiting
Less common: Ataxia, blurred vision, rash
Serious: Stevens-Johnson syndrome, blood disorders, liver dysfunction
Pregnancy: Use only if clearly necessary; potential risk to fetus
Lactation: Use with caution; excreted in breast milk
Regular monitoring of blood counts and liver function essential
Advise patients to report rash or hypersensitivity symptoms immediately
Avoid sudden discontinuation to prevent seizures
Anticonvulsant / Antiepileptic
