Indications:
Hypertension, stable angina, and moderate to severe chronic heart failure (not for hypertensive emergencies)
Pharmacology:
Highly selective β₁‑adrenergic blocker; reduces heart rate and cardiac output, thereby lowering blood pressure without adversely affecting lipid metabolism
Dosage:
Hypertension/Angina: Usually starts at 5 mg once daily; 2.5 mg may be used initially in some patients. Maintenance up to 10‑20 mg daily per clinical response
Heart Failure: Initial 1.25 mg/day, titrated weekly (2.5 → 3.75 → 5 → 7.5 →10 mg) up to max 10 mg/day
Administration:
Oral, with or without food; can be taken any time of day
Interaction:
May enhance effects of other antihypertensives; caution with CYP2D6/3A4 inhibitors, digitalis, Class I antiarrhythmics, and NSAIDs. Hypoglycemia signs may be masked in diabetic patients. Avoid abrupt withdrawal to reduce risk of angina or rebound hypertension
Contraindications:
Cardiogenic shock, overt cardiac failure, second/third-degree AV block, marked sinus bradycardia, severe bronchospastic disease
Side Effects:
Common: dizziness, fatigue, bradycardia, headache, cold extremities, gastrointestinal symptoms, sleep disturbances. Rare: bronchospasm, depression .
Pregnancy & Lactation:
Not recommended unless clearly needed; may reduce uteroplacental blood flow. Excreted in breast milk—use caution
Precautions & Warnings:
Use cautiously in asthma, diabetes, renal/hepatic impairment, AV conduction disorders, peripheral vascular disease. Monitor on discontinuation and for orthostatic hypotension .

Store below 30 °C in a cool, dry place; protect from light and moisture. Keep out of reach of children .
