Indications:
Ciprocin is indicated for the treatment of infections caused by susceptible bacteria, including:
Urinary tract infections (UTIs)
Respiratory tract infections (including pneumonia)
Skin and soft tissue infections
Bone and joint infections
Gastrointestinal infections (e.g., infectious diarrhea)
Certain sexually transmitted infections
Pharmacology:
Ciprofloxacin is a fluoroquinolone antibiotic that inhibits bacterial DNA gyrase and topoisomerase IV, enzymes critical for bacterial DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.
Dosage & Administration:
Adults: 250–750 mg every 12 hours depending on infection severity
Tablets should be taken 2 hours before or 4 hours after antacids, sucralfate, or mineral supplements (iron, zinc, calcium) to avoid reduced absorption
Take with a full glass of water; can be taken with or without food
Interaction:
Antacids, sucralfate, and mineral supplements reduce absorption
May increase effects of warfarin and theophylline
Avoid concurrent use with tizanidine
May interact with cyclosporine and phenytoin
Contraindications:
Hypersensitivity to ciprofloxacin or other fluoroquinolones
Tendon disorders related to fluoroquinolone use
Children and adolescents (risk of musculoskeletal adverse effects, unless benefits outweigh risks)
Side Effects:
Common: Nausea, diarrhea, dizziness, headache, abdominal pain
Serious: Tendon rupture, QT prolongation, hypersensitivity reactions, peripheral neuropathy, CNS effects (seizures, tremors)
Pregnancy & Lactation:
Use only if clearly needed during pregnancy; animal studies showed adverse effects on cartilage. Ciprofloxacin is excreted in breast milk; breastfeeding is generally not recommended during treatment.
Precautions & Warnings:
Increased risk of tendonitis and tendon rupture, especially in elderly and corticosteroid users
Avoid excessive sunlight or UV exposure (photosensitivity)
Monitor for signs of neuropathy or CNS toxicity
Use cautiously in renal impairment with dose adjustment
