Indications:
Phylopen DS is indicated for the treatment of infections caused by penicillinase-producing staphylococci and other susceptible organisms. These include:

  • Skin and soft tissue infections

  • Bone and joint infections

  • Respiratory tract infections

  • Wound infections

  • Post-surgical infections

  • Otitis media and externa

Pharmacology:
Flucloxacillin is a narrow-spectrum, beta-lactamase-resistant penicillin. It works by inhibiting bacterial cell wall synthesis, leading to cell lysis and death. It is effective particularly against penicillin-resistant staphylococcal infections. It is stable in the presence of beta-lactamase enzymes.

Dosage & Administration:

  • Adults: 250–500 mg every 6 hours (four times daily), taken 30–60 minutes before meals.

  • Severe infections: Dose may be increased under physician supervision.

  • Children (over 2 years): Half the adult dose.
    Capsules should be taken on an empty stomach for optimal absorption.

Interaction:
May reduce the efficacy of oral contraceptives. May increase the risk of nephrotoxicity when used with aminoglycosides or other nephrotoxic agents. May interact with methotrexate (reduced clearance).

Contraindications:
Contraindicated in patients with:

  • History of hypersensitivity to penicillins or cephalosporins

  • History of flucloxacillin-associated jaundice or hepatic dysfunction

Side Effects:
Common: Nausea, diarrhea, skin rash
Rare but serious: Hepatitis, cholestatic jaundice, anaphylaxis, Stevens-Johnson syndrome
Long-term use may lead to superinfection or antibiotic-associated colitis.

Pregnancy & Lactation:
Considered safe in pregnancy (Category B). Use during breastfeeding is generally safe but should be monitored, especially for allergic reactions in infants.

Precautions & Warnings:
Monitor liver function during prolonged therapy. Caution in patients with renal or hepatic impairment. Ensure full course is completed to prevent resistance. Do not use for viral infections.
