Indications:
Comet is indicated for the management of type 2 diabetes mellitus, especially in overweight patients when dietary management and exercise alone do not result in adequate glycemic control. It may be used as monotherapy or in combination with other antidiabetic agents.
Pharmacology:
Comet contains Metformin Hydrochloride, a biguanide class oral antihyperglycemic agent. It works by decreasing hepatic glucose production, reducing intestinal glucose absorption, and improving insulin sensitivity by increasing peripheral glucose uptake and utilization.
Dosage & Administration:
Initial dose: 500 mg once or twice daily with meals.
Maximum recommended daily dose: 2000–2550 mg (divided doses).
Dosage should be individualized based on patient’s response and tolerance.
Interaction:
May interact with alcohol (increased risk of lactic acidosis), cimetidine, corticosteroids, diuretics, and contrast agents. Use with caution.
Contraindications:
Renal impairment (eGFR <30 mL/min/1.73m²)
Acute or chronic metabolic acidosis
Severe hepatic dysfunction
Known hypersensitivity to Metformin
Side Effects:
Common side effects include gastrointestinal symptoms like nausea, vomiting, diarrhea, abdominal pain, and loss of appetite. Rare but serious side effect: lactic acidosis.
Pregnancy & Lactation:
Generally considered safe in pregnancy for women with gestational diabetes under medical supervision. Excreted in breast milk in small amounts; caution advised during lactation.
Precautions & Warnings:
Monitor renal function before starting and regularly during treatment.
Avoid alcohol and iodinated contrast imaging during therapy.
Discontinue in cases of dehydration or serious infection.
