Indications:
Perkinil is indicated for:
Idiopathic Parkinson’s disease
Post-encephalitic and arteriosclerotic parkinsonism
Drug-induced extrapyramidal symptoms (caused by antipsychotic or neuroleptic medications)
Muscle rigidity, tremors, and akathisia
Pharmacology:
Trihexyphenidyl is a centrally acting anticholinergic agent that works by blocking acetylcholine receptors in the brain. This action helps restore the balance between acetylcholine and dopamine, improving muscle control and reducing involuntary movements.
Dosage & Administration:
Adults (Parkinsonism): Start with 1 mg/day; gradually increase by 2 mg increments every 3–5 days up to 6–10 mg/day in divided doses.
Drug-induced extrapyramidal symptoms: 1–4 mg/day in divided doses.
Should be taken with food to minimize gastric irritation.
Interaction:
May increase anticholinergic side effects if used with antihistamines, tricyclic antidepressants, or other anticholinergics
Reduces the effectiveness of levodopa in some patients
Alcohol may enhance CNS depression
Contraindications:
Hypersensitivity to Trihexyphenidyl
Narrow-angle glaucoma
Obstructive diseases of the gastrointestinal or genitourinary tract
Side Effects:
Dry mouth, blurred vision
Drowsiness, dizziness
Constipation, urinary retention
Nervousness, confusion (especially in elderly)
Pregnancy & Lactation:
Use with caution. Safety during pregnancy and lactation has not been fully established—should be used only if clearly needed.
Precautions & Warnings:
Use cautiously in elderly and patients with cardiac, liver, or kidney disease
Monitor for signs of cognitive changes or hallucinations
Avoid sudden discontinuation
