Ardance-M 5 mg + 500 mg Tablet is a fixed-dose combination medication manufactured by Pharmacil Limited, combining two active ingredients: Empagliflozin (5 mg) and Metformin Hydrochloride (500 mg). This formulation is primarily used to manage type 2 diabetes mellitus in adults, especially when diet and exercise alone do not provide adequate glycemic control.
Empagliflozin is a Sodium-Glucose Cotransporter-2 (SGLT2) inhibitor that works by reducing glucose reabsorption in the kidneys, leading to increased glucose excretion in urine. Metformin is a biguanide that decreases hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization. Together, these medications help lower blood glucose levels through complementary mechanisms.
The recommended starting dose is one tablet twice daily with meals, to minimize gastrointestinal side effects associated with Metformin. The maximum daily dose should not exceed 25 mg of Empagliflozin and 2000 mg of Metformin. Before initiating treatment, renal function should be assessed, and the combination is contraindicated in patients with an estimated glomerular filtration rate (eGFR) below 45 mL/min/1.73 m².
Common side effects include gastrointestinal disturbances such as nausea, vomiting, and abdominal discomfort, as well as urinary tract infections and genital fungal infections. Serious but rare side effects may include lactic acidosis, a medical emergency requiring immediate attention.
