Indications:
Duovas is indicated for patients requiring concurrent treatment of hypertension and dyslipidemia. It helps reduce cardiovascular risk in patients with risk factors such as diabetes, coronary artery disease, or a history of stroke or myocardial infarction.
Pharmacology:
Amlodipine is a dihydropyridine calcium channel blocker that inhibits calcium ion influx in vascular smooth muscle and cardiac muscle. It lowers blood pressure by causing vasodilation.
Atorvastatin is an HMG-CoA reductase inhibitor (statin) that reduces cholesterol synthesis in the liver, lowering total cholesterol, LDL-C, and triglycerides while modestly increasing HDL-C.
Dosage & Administration:
The usual adult dose is one tablet (5 mg Amlodipine + 20 mg Atorvastatin) once daily.
Dose adjustments may be required based on patient response, renal or hepatic function.
Can be taken with or without food, preferably at the same time each day.
Interaction:
Amlodipine: May interact with other antihypertensives, CYP3A4 inhibitors (e.g., diltiazem), and grapefruit juice.
Atorvastatin: May interact with antifungals (e.g., ketoconazole), macrolide antibiotics, cyclosporine, digoxin, and warfarin. Risk of myopathy increases with fibrates or niacin.
Contraindications:
Hypersensitivity to any component
Active liver disease or unexplained persistent elevation of liver enzymes
Pregnancy and lactation (especially due to statin component)
Severe hypotension, aortic stenosis, or cardiogenic shock
Side Effects:
Common: Headache, dizziness, ankle swelling (from Amlodipine); muscle pain, GI upset (from Atorvastatin)
Serious (rare): Rhabdomyolysis, liver enzyme elevation, allergic reactions, hypotension
Pregnancy & Lactation:
Contraindicated in pregnancy and during breastfeeding due to the Atorvastatin component. Statins can harm fetal development. Effective contraception is advised during use.
Precautions & Warnings:
Monitor liver function tests and lipid profiles during therapy
Caution in patients with renal or hepatic impairment
Avoid alcohol and grapefruit juice
Amlodipine may cause peripheral edema; monitor BP closely
