Indications:
Exium is indicated for the treatment of:
Gastroesophageal reflux disease (GERD)
Erosive esophagitis
Peptic ulcer disease
Zollinger-Ellison syndrome
NSAID-associated gastric ulcers
H. pylori eradication (as part of combination therapy)
Pharmacology:
Exium contains Esomeprazole, the S-isomer of omeprazole, which is a proton pump inhibitor (PPI). It inhibits the H+/K+ ATPase enzyme in gastric parietal cells, reducing gastric acid secretion and promoting healing of ulcers and erosions.
Dosage & Administration:
Usual adult dose: 20 mg once daily for 4 to 8 weeks, depending on condition
Take at least 1 hour before meals
Swallow whole with water—do not chew or crush the tablet
Interaction:
May interact with:
Clopidogrel (may reduce its effect)
Ketoconazole, itraconazole (reduced absorption)
Warfarin, diazepam, phenytoin (increased plasma levels)
Contraindications:
Hypersensitivity to Esomeprazole or any PPI
Severe liver impairment (use cautiously)
Side Effects:
Common: Headache, diarrhea, abdominal pain, nausea, flatulence
Rare: Vitamin B12 deficiency (long-term use), hypomagnesemia, bone fractures, skin rash
Pregnancy & Lactation:
Pregnancy Category B: No proven risk in humans.
Safe during breastfeeding in usual doses, but caution is advised.
Precautions & Warnings:
Do not use continuously for long periods without doctor’s advice
Evaluate for gastric malignancy before starting therapy in case of alarming symptoms
Monitor magnesium and vitamin B12 in long-term use
