Indications:
Triginal MR 35 mg Tablet is indicated for:

  • Symptomatic treatment of stable angina pectoris in adults inadequately controlled by or intolerant to first-line anti-anginal therapies.

Pharmacology:
Trimetazidine is a metabolic anti-ischaemic agent that inhibits the enzyme 3-keto acyl CoA thiolase, shifting myocardial energy production from fatty acid oxidation to glucose oxidation. This shift enhances energy production efficiency, reduces oxygen consumption, and improves myocardial function during ischemic conditions.

Dosage & Administration:

  • Adults: 35 mg twice daily or 20 mg three times daily during meals.

  • The treatment's effectiveness should be evaluated after three months; discontinue if no benefit is observed.

Interaction:

  • No significant drug interactions have been reported with beta-blockers, calcium antagonists, nitrates, heparin, hypolipidemic agents, or digitalis preparations.

Contraindications:

  • Hypersensitivity to Trimetazidine or any of its excipients.

  • Parkinson's disease, parkinsonian symptoms, tremors, restless legs syndrome, or severe renal impairment.

Side Effects:
Common side effects include:

  • Dizziness, headache

  • Abdominal pain, diarrhea, dyspepsia

  • Nausea, vomiting

  • Rash, pruritus, urticaria

  • Asthenia (weakness)

Pregnancy & Lactation:

  • Limited data on the use of Trimetazidine during pregnancy; animal studies do not indicate direct or indirect harmful effects. As a precaution, its use during pregnancy is not recommended.

  • It is unknown whether Trimetazidine is excreted in human milk; due to potential risks to the newborn, its use during breastfeeding is contraindicated.

Precautions & Warnings:

  • Not a curative treatment for acute angina attacks or unstable angina pectoris.

  • Not indicated for the treatment of myocardial infarction.

  • Use with caution in elderly patients and those with renal or hepatic impairment.

  • Monitor for signs of Parkinsonism, especially in elderly patients.
