Indications:
Treatment of hypertension, angina pectoris, and chronic heart failure, either alone or in combination therapy
Pharmacology:
Bisoprolol is a highly selective β₁‑adrenergic blocker. It decreases heart rate, cardiac output, and reduces blood pressure
Dosage:
Hypertension / Angina: Usually starts 5 mg once daily; in some patients 2.5 mg is appropriate. Max dose 20 mg daily
Heart Failure: Initiate at 1.25 mg once daily, titrate weekly up to 10 mg daily
Adjust dose in renal/hepatic impairment; starting 2.5 mg daily
Administration:
Oral use with or without food; swallow whole
Interaction:
May interact with other antihypertensives; caution when combined with CYP2D6/3A4 inhibitors. Use caution in diabetic patients due to masked hypoglycemia; dizziness risk when used with other CNS depressants
Contraindications:
Cardiogenic shock, overt heart failure, second/third‑degree AV block, marked sinus bradycardia, severe asthma/pulmonary disease
Side Effects:
Common: fatigue, dizziness, headache, bradycardia, gastrointestinal symptoms, sleep disturbances, peripheral vasoconstriction .Rare: bronchospasm, worsening heart failure.
Pregnancy & Lactation:
Safety not well established. Use only if clearly needed and prescribed by a physician
Precautions & Warnings:
Taper gradually—abrupt withdrawal may precipitate angina or hypertensive rebound. Use with caution in diabetes, thyroid disorders, renal/hepatic impairment, and respiratory diseases .
