Description

Daomin XR 500 mg is an extended-release formulation of Metformin Hydrochloride, developed by ACME Laboratories Ltd. It is primarily used to manage Type 2 diabetes mellitus, particularly in overweight patients whose blood glucose levels are inadequately controlled by diet and exercise alone. Metformin works by reducing hepatic glucose production, decreasing intestinal glucose absorption, and enhancing insulin sensitivity, thereby lowering both basal and postprandial plasma glucose levels. The extended-release (XR) formulation allows for once-daily dosing, improving patient adherence and minimizing gastrointestinal side effects compared to immediate-release versions. The usual starting dose is 500 mg once daily with the evening meal, with potential dose increases in 500 mg increments weekly, up to a maximum of 2000 mg once daily. Patients previously on immediate-release metformin can be switched to Daomin XR at the same total daily dose, up to 2000 mg once daily. It's important to note that Daomin XR should not be crushed or chewed. As with all metformin formulations, it is contraindicated in patients with severe renal impairment (eGFR below 30 mL/min/1.73 m²) and should be used with caution in those with moderate renal impairment. Regular monitoring of renal function is recommended during treatment. Additionally, metformin is not recommended for use during pregnancy; insulin is preferred for managing blood glucose levels during pregnancy. While metformin is excreted into human breast milk, no adverse effects have been observed in breastfed infants, but a decision should be made whether to discontinue breastfeeding or discontinue the drug, considering the importance of the drug to the mother. Patients should also be aware of the potential risk of lactic acidosis, a rare but serious complication associated with metformin use. Symptoms of lactic acidosis include muscle pain, respiratory distress, and abdominal discomfort. If these occur, immediate medical attention is required. Patients should be monitored regularly, especially during the initial weeks of treatment, to adjust the dosage appropriately and ensure optimal glycemic control.
