Indications:
Maxima Tablet is indicated for the treatment of gastroesophageal reflux disease (GERD), erosive esophagitis, NSAID-associated gastric ulcer, Zollinger-Ellison syndrome, and as part of combination therapy for Helicobacter pylori eradication.
Pharmacology:
Esomeprazole is a proton pump inhibitor (PPI) that selectively inhibits the H⁺/K⁺ ATPase enzyme system of the gastric parietal cells. This inhibition blocks the final step of gastric acid production, leading to reduced acidity and improved healing of acid-related damage.
Dosage & Administration:
Recommended dose is 20 mg once daily, taken at least 1 hour before meals. Duration of treatment depends on the clinical condition—typically 4 to 8 weeks for GERD and ulcers. The tablet should be swallowed whole with water and not chewed or crushed.
Interaction:
Esomeprazole may reduce the absorption of drugs that require an acidic environment (e.g., ketoconazole, itraconazole). It can interact with warfarin, diazepam, phenytoin, and clopidogrel. Avoid concurrent use with atazanavir and nelfinavir.
Contraindications:
Contraindicated in patients with hypersensitivity to Esomeprazole or other substituted benzimidazoles. Not recommended in patients with severe hepatic impairment without monitoring.
Side Effects:
Common side effects include headache, nausea, flatulence, abdominal pain, diarrhea, and constipation. Long-term use may lead to vitamin B12 deficiency, hypomagnesemia, and bone fractures.
Pregnancy & Lactation:
Pregnancy Category B. Safe use during pregnancy and lactation should be based on risk-benefit evaluation by a physician.
Precautions & Warnings:
Use with caution in long-term therapy. May mask symptoms of gastric cancer. Regular monitoring is advised in prolonged use and in patients with liver disease, osteoporosis, or low magnesium levels.
