Indications:
Maxima is indicated for the treatment of gastroesophageal reflux disease (GERD), erosive esophagitis, peptic ulcers (including those associated with NSAIDs), and Zollinger-Ellison Syndrome. It is also used for Helicobacter pylori eradication therapy in combination with antibiotics.
Pharmacology:
Esomeprazole is the S-isomer of omeprazole and a proton pump inhibitor (PPI). It inhibits the H⁺/K⁺ ATPase enzyme in the gastric parietal cells, thereby blocking the final step in acid production and reducing gastric acidity. This promotes healing of acid-related damage and relieves symptoms.
Dosage & Administration:
The usual adult dose is 20 mg once daily for 4–8 weeks, depending on the condition. For maintenance therapy or prevention of relapse, the dose may be continued as directed by a physician. The capsule should be taken at least one hour before meals and swallowed whole with water.
Interaction:
May interact with drugs like clopidogrel (reduces its effectiveness), diazepam, phenytoin, warfarin, and ketoconazole. Concomitant use with atazanavir or nelfinavir is not recommended.
Contraindications:
Contraindicated in patients with known hypersensitivity to Esomeprazole, other PPIs, or any of the excipients. Avoid use in patients with severe hepatic impairment without proper monitoring.
Side Effects:
Common side effects include headache, abdominal pain, constipation, diarrhea, nausea, and flatulence. Long-term use may lead to vitamin B12 deficiency, hypomagnesemia, or increased risk of fractures.
Pregnancy & Lactation:
Classified as Pregnancy Category B. Use during pregnancy or lactation only if clearly needed and prescribed by a physician.
Precautions & Warnings:
Use with caution in patients with liver disease, osteoporosis, or those at risk of electrolyte imbalances. Prolonged use should be monitored due to risk of gastric malignancy masking, and routine magnesium levels may be required in long-term use.
