Indications:
ARNIGEN 100MG is indicated for:

  • Reducing the risk of cardiovascular death and hospitalization due to heart failure in patients with chronic heart failure (NYHA Class II-IV) and reduced ejection fraction.

  • Treating symptomatic heart failure with systemic left ventricular systolic dysfunction in pediatric patients aged one year and older.

Pharmacology:

  • Sacubitril is a prodrug that is metabolized to its active form, LBQ657, which inhibits neprilysin. Neprilysin normally breaks down natriuretic peptides like ANP and BNP. Inhibition leads to increased levels of these peptides, promoting vasodilation, natriuresis, and diuresis.

  • Valsartan is an angiotensin II receptor blocker (ARB) that blocks the effects of angiotensin II, leading to vasodilation and reduced blood pressure.

Dosage:

  • Initial Dose: 49 mg Sacubitril / 51 mg Valsartan twice daily.

  • Maintenance Dose: After 2 to 4 weeks, the dose may be increased to 97 mg Sacubitril / 103 mg Valsartan twice daily, as tolerated by the patient.

Administration:

  • Take orally, with or without food.

  • Swallow tablets whole; do not crush or chew.

Interaction:

  • Avoid concomitant use with ACE inhibitors; a 36-hour washout period is required when switching between these medications.

  • Use caution when combining with aliskiren in patients with diabetes.

Contraindications:

  • Hypersensitivity to Sacubitril, Valsartan, or any component of the formulation.

  • History of angioedema related to previous ACE inhibitor or ARB therapy.

  • Concomitant use with ACE inhibitors or aliskiren in patients with diabetes.

Side Effects:

  • Common: Angioedema, hypotension, impaired renal function, hyperkalemia, cough, dizziness.

  • Serious: Severe hypotension, renal impairment, hyperkalemia, and angioedema.

Pregnancy & Lactation:

  • Pregnancy: Contraindicated during pregnancy due to potential fetal harm.

  • Lactation: Not recommended during breastfeeding; consult a physician before use.

Therapeutic Class:
Combined antihypertensive preparations.

Storage Conditions:

  • Store in a dry place below 30°C.

  • Protect from moisture and keep out of the reach of children.
