Indications:

Fenolid is indicated for the treatment of:

  • Hypercholesterolemia (type IIa, IIb)

  • Mixed hyperlipidemia (type III)

  • Hypertriglyceridemia (type IV, V)

It is used in patients who have not responded adequately to diet and other appropriate measures.

Pharmacology:

Fenolid contains fenofibrate, a fibric acid derivative. It is rapidly hydrolyzed to its active form, fenofibric acid, which activates peroxisome proliferator-activated receptor alpha (PPARα). This activation leads to increased lipoprotein lipase activity and decreased secretion of apolipoprotein C-III, resulting in enhanced clearance of triglyceride-rich lipoproteins from the bloodstream. Additionally, fenofibrate increases HDL cholesterol levels by increasing the synthesis of apolipoproteins AI and AII.

Dosage & Administration:

  • Adults with hypercholesterolemia or mixed hyperlipidemia: The initial dose is 200 mg once daily.

  • Adults with hypertriglyceridemia: The initial dose is between 67 to 200 mg per day.

  • Fenolid should be taken with food to enhance absorption.

Interaction:

  • Warfarin: Fenolid may potentiate the anticoagulant effects of warfarin.

  • Antidiabetic drugs: May improve glucose tolerance and have an additive effect.

  • Cyclosporine: May increase nephrotoxicity.

  • Statins: Caution is advised due to potential increased risk of rhabdomyolysis; however, low-dose statins with Fenolid appear to be well tolerated.

Contraindications:

  • Hypersensitivity to fenofibrate or any of its components.

  • Severe renal impairment.

  • Severe hepatic impairment.

  • Gallbladder disease.

  • Breastfeeding.

Side Effects:

  • Common: Diarrhea, stomach pain, nausea, vomiting, flatulence, elevated liver enzymes.

  • Uncommon: Headache, gallstones, reduced sex drive, rash, itching, increased creatinine levels.

  • Serious (rare): Muscle pain, weakness, or cramps (myopathy), pancreatitis, blood clots (deep vein thrombosis or pulmonary embolism), liver inflammation (hepatitis), allergic reactions.

Pregnancy & Lactation:

  • Pregnancy: Fenolid should only be used during pregnancy after a careful benefit/risk assessment.

  • Lactation: It is unknown whether fenofibrate and/or its metabolites are excreted in human milk. A risk to the suckling child cannot be excluded. Therefore, fenofibrate should not be used during breastfeeding.
