Sugamet 500mg is indicated as an adjunct to diet and exercise to improve glycemic control in patients with type 2 diabetes mellitus. It is also used in combination with other oral hypoglycemic agents or insulin when diet and exercise alone do not provide adequate glycemic control.
Pharmacology:
Metformin, the active ingredient in Sugamet, is a biguanide class oral antihyperglycemic drug. It works by:
Decreasing hepatic glucose production
Decreasing intestinal absorption of glucose
Improving insulin sensitivity by increasing peripheral glucose uptake and utilization
Unlike some other antidiabetic medications, Metformin does not cause hypoglycemia when used alone.
Dosage & Administration:
Adults: The usual starting dose is 500 mg twice a day or 850 mg once a day, taken with meals. Dosage may be increased gradually, up to a maximum of 2000 mg per day, based on patient tolerance and glycemic control.
Children: The usual starting dose is 500 mg twice a day, taken with meals. Dosage increases should be made in increments of 500 mg weekly, up to a maximum of 2000 mg per day, given in divided doses.
Interaction:
Co-administration with carbonic anhydrase inhibitors (e.g., topiramate, zonisamide) may increase the risk of lactic acidosis.
Drugs that reduce Metformin clearance (e.g., cimetidine) may increase the accumulation of Metformin.
Alcohol can potentiate the effect of Metformin on lactate metabolism, increasing the risk of lactic acidosis.
Contraindications:
Severe renal impairment (eGFR below 30 mL/min/1.73 m²)
Hypersensitivity to Metformin or any component of the formulation
Acute or chronic metabolic acidosis, including diabetic ketoacidosis

Pregnancy & Lactation:
Pregnancy: Uncontrolled diabetes during pregnancy is associated with an increased risk of congenital abnormalities and perinatal mortality. Insulin is preferred for managing diabetes during pregnancy.
Breastfeeding: Metformin is excreted into human breast milk. No adverse effects were observed in breastfed newborns/infants. However, a decision on whether to discontinue breastfeeding should be made, taking into account the benefit of breastfeeding and the potential risk to adverse effects on the child.
