Indications:
PRESART 80 mg is indicated for:
Treatment of essential hypertension
Reduction of cardiovascular morbidity in patients with atherothrombotic cardiovascular disease (e.g., coronary artery disease, stroke)
Patients with type 2 diabetes and documented target organ damage
Pharmacology:
Telmisartan is a long-acting angiotensin II receptor blocker (ARB) that specifically blocks the AT1 receptor. This prevents angiotensin II from binding, leading to vasodilation and reduced blood pressure. It does not inhibit ACE or affect bradykinin levels, thus reducing side effects like cough.
Dosage:
Hypertension: 40 mg once daily; may increase to 80 mg once daily as needed.
Cardiovascular risk reduction: 80 mg once daily.
Administration:
Take orally once a day with or without food.
Take at the same time each day for consistent blood pressure control.
Interaction:
Increased effects: May occur with digoxin, potassium supplements, potassium-sparing diuretics
Decreased effects: Rifampicin, NSAIDs may reduce efficacy
Caution with lithium: Risk of lithium toxicity
Avoid concurrent use with aliskiren in diabetic or renal-impaired patients
Contraindications:
Hypersensitivity to Telmisartan
Pregnancy and lactation
Severe hepatic impairment or biliary obstruction
Concomitant use with aliskiren in certain patient populations
Side Effects:
Common: Dizziness, back pain, sinusitis, diarrhea, fatigue
Rare: Angioedema, hypotension, renal impairment, increased liver enzymes
Seek immediate attention for signs of allergic reaction or severe hypotension
Pregnancy & Lactation:
Pregnancy: Contraindicated, especially in 2nd and 3rd trimesters. May harm fetal development.
Lactation: Unknown if excreted in human milk. Avoid or consult physician before use.
