Indications:
PROVAL CR 500 mg is indicated for:
Treatment of all types of epilepsy, including partial seizures, absence seizures (petit mal), generalized tonic-clonic seizures (grand mal), myoclonic seizures, atonic seizures, and mixed seizures with absence attacks.
Management of manic episodes associated with bipolar disorder.
Prophylaxis of migraine headaches.
Pharmacology:
Sodium Valproate, the active ingredient of this preparation, has anti-epileptic activity against a variety of seizures. The mechanism by which Sodium Valproate exerts its anti-epileptic effects has not been fully established. However, it is believed to increase brain levels of gamma-aminobutyric acid (GABA), which helps in controlling seizures.
Dosage & Administration:
Epilepsy:
Adults: Initially, 600 mg daily in 2 divided doses, preferably after food. The dose may be increased by 200 mg/day at 3-day intervals to a maximum of 2.5 g daily in divided doses until control of seizures is achieved. The usual maintenance dose is 1–2 g daily (20–30 mg/kg daily).
Children (over 20 kg): Initially 400 mg daily in divided doses, increased until control (usually in the range of 20–30 mg/kg daily); maximum 35 mg/kg daily.
Children (up to 20 kg): Initially 20 mg/kg daily in divided doses.
Febrile Convulsions: 20–30 mg/kg/day in 3 divided doses.
Bipolar Disorder: Initially 600 mg daily (20–30 mg/kg/day) in 2–3 divided doses. The maintenance dosage is 1000–2000 mg daily.
Migraine Prophylaxis: 400–600 mg daily, although some may require 1000–1500 mg daily.
Administration:
Take orally with or without food, at the same time each day. Swallow tablets whole; do not chew or crush.
Interaction:
Sodium Valproate may interact with other anticonvulsants (e.g., phenobarbital, phenytoin), anticoagulants (e.g., warfarin), and certain antidepressants. Monitor for potential interactions and adjust dosages accordingly.
Contraindications:
Hypersensitivity to Sodium Valproate or any component of the formulation.
Severe liver dysfunction.
Urea cycle disorders.
Mitochondrial disorders.
Side Effects:
Common: Nausea, vomiting, dizziness, drowsiness, tremors, weight gain, hair loss.
Less Common: Abdominal pain, diarrhea, blurred vision, rash, increased appetite.
Serious: Hepatic dysfunction, pancreatitis, thrombocytopenia, hyperammonemia.
Pregnancy & Lactation:
Pregnancy: Contraindicated, especially during the first trimester, due to potential teratogenic effects.
Lactation: Not recommended; excreted in breast milk and may affect the nursing infant.
