Indications:
Presart 20 mg is indicated for the treatment of:
Essential hypertension in adults
Reduction of cardiovascular morbidity in adults with:
A history of coronary heart disease
Stroke
Peripheral arterial disease
Type 2 diabetes mellitus with documented target organ damage
Pharmacology:
Telmisartan is a non-peptide Angiotensin-II receptor blocker (ARB). Angiotensin-II is a potent vasoconstrictor formed from angiotensin-I by the action of angiotensin-converting enzyme (ACE). Telmisartan selectively blocks the binding of angiotensin-II to the AT1 receptor in various tissues, such as vascular smooth muscle and the adrenal gland, thereby inhibiting its vasoconstrictor and aldosterone-secreting effects. This action leads to vasodilation and a reduction in blood pressure. Telmisartan has a much greater affinity (>3,000-fold) for the AT1 receptor than for the AT2 receptor.
Dosage & Administration:
Hypertension:
The usual starting dose is 40 mg once daily. The dose can range from 20 to 80 mg once daily, depending on the patient's response.
Most antihypertensive effects are observed within 2 weeks, with maximal reduction generally attained after 4 weeks.
Cardiovascular Risk Reduction:
The recommended dose is 80 mg once daily.
Administration:
Presart 20 mg tablets may be taken with or without food.
The tablet should be swallowed whole with a glass of water.
It is advisable to take the tablet at the same time each day to maintain consistent blood levels.
Interaction:
NSAIDs: Concurrent use may increase the risk of renal impairment and reduce the antihypertensive effect.
Potassium Supplements or Potassium-Sparing Diuretics: May increase the risk of hyperkalemia.
Digoxin: Co-administration may increase digoxin plasma concentrations; monitor digoxin levels.
Contraindications:
Known hypersensitivity to telmisartan or any component of the formulation.
Pregnancy (especially during the second and third trimesters).
Severe hepatic impairment.
Concomitant use with aliskiren in diabetic patients.
Side Effects:
Common side effects include:
Upper respiratory infections
Back pain
Diarrhea
Sinusitis
Dizziness
Serious side effects may include:
Hypotension
Hyperkalemia
Renal dysfunction
Pregnancy & Lactation:
Pregnancy: Category C (first trimester); Category D (second and third trimesters). Use is contraindicated during the second and third trimesters due to potential fetal and neonatal harm.
Lactation: It is not known whether telmisartan is excreted in human milk. Caution should be exercised when administered to a nursing woman.
Precautions & Warnings:
Use with caution in patients with renal or hepatic impairment.
Monitor blood pressure, renal function, and electrolytes periodically during treatment.
Avoid concomitant use with ACE inhibitors in patients with diabetes.
Overdose Effects:
Overdosage may lead to hypotension, dizziness, tachycardia, bradycardia, and renal failure. In case of overdose, supportive treatment should be initiated.
